Registration update

OCPW is still accepting registrations for football and cheerleading. Please register now to avoid the increase in registration fee. After June 15th, registration fees will go up to $230/participant (to offset the costs for placing extra orders). All registrations must be received by July 15th (no exceptions).

Original birth certificate verification must be arranged so please contact us to arrange it. All fees must accompany all forms. There are three (3) forms that must be signed by the participant so please double-check your forms. Physicals must be filled out by a physician and must have been performed since Jan. 1, 2006 (no exceptions).
