    REGISTRATION IS STILL OPEN

    It is not too late to register for football or cheer
    There are still spaces available
    All forms, copies, and fees need to be downloaded, completed, and mailed in as soon as possible. The verification of the original birth certificate can be done at equipment pickup.
    The physical form may be the only other item that you may need to bring in if you need to get the form completed by the MD (a substitute form will not be accepted).
    If fees cannot be paid all at once, please contact us immediately so that we can make arrangements.
    ***For girls 8-11 interested in cheer, if you plan to register, please contact us immediately (additional uniforms may need to be ordered).

    EQUIPMENT & UNIFORM PICK UP DATES

    Equipment & uniforms can be picked up on any one of these dates (2124 Meares Rd, Chapel Hill 27514):
    • July 25th from 6-8 pm
    • July 26th from 10:00 am-2:00 pm
    • July 27th from 1-3 pm

    Your participant must come with you to pick up uniforms and equipment. Pictures for player/cheerleader/coach IDs will be taken.

    JULY 13TH REGISTRATION

    DON'T FORGET: Registration for 2008 season on Sunday, July 13th, from 1:30-3:30 pm at Cane Creek Baptist Activities Center. All information and forms are located under "Registration". Make sure that you have all or most of the following items to get registered:
    • 2 copies of birth certificate
    • original birth certificate (for verification--must have raised seal)
    • 2 copies of final 2008-2009 report cards (complete 4 quarters: if more than one school during school year, bring 2 copies of all)
    • PW Participant Contract & Parental Consent form (2 pages--second page must be signed by participant)
    • PW Medical and Physical form (2 pages--second page must be completed by MD, no substitute forms will be accepted)
    • OCPW Medical Release & Waiver of Liability form
    • OCPW Email/Photo Permission form
    • OCPW Payment & Refund form
    • $125 registration fee
    • $25 spirit pack fee
