All Football Teams and Cheerleading Squads are formed and filled on a first-come, first-serve basis. The participant will not be allowed to participate until all required forms have been submitted.
Registration fee for 2009 season is $100/participant. There is a sibling discount (the fee will decrease in $15 increments for each additional child: first child's fee is $100, second child's fee is $85, third child's fee is $70, etc). Payment plans available. OCPW now accepts credit cards through Paypal; however, you must add $5 to total amount to use this option.
NOTE: the registration fee does not include the mandatory spirit pack fee. There is a $25 service fee for all returned checks and all future payment must be cash.
A spirit pack ($25) is mandatory and must be purchased at time of registration of each child (football includes t-shirt, shorts, socks, mouthpiece; cheer includes t-shirt, shorts, socks, hair bow). Register by the May 16th registration in order to receive a free spirit. REGISTER EARLY TO GET THIS GREAT DEAL!!!
Two copies of Birth Certificates (and must show OCPW board member original birth certificate for verification)
Two copies of final 2008-2009 Report Card (if beginning kindergarten for the 2009-2010 school year, contact OCPW for instructions)
2009 Pop Warner Participant Contract & Parental Consent Form (must be dated and signed by both the parent and participant)
2009 Pop Warner Physical & Medical Form (No substitution forms accepted, must be dated after Jan 1, 2009 by physician)
